Please take a moment to review the following information

Consultation

I offer a free 30-minute consultation (either on the phone or in my office), so you can get a sense of whether we have a good match. It is also an opportunity for me to decide whether or not I will be able to help you.

Reduced Fees

Many people understand that it is going to take quite a few meetings with the therapist to reach their desired goals. Therefore, reduced fees are available through the purchase of session packages (either 5 or 10) in bulk. If you request, a Payment Options Handout can be sent to you by email.

Rates

Either during a Consultation or when you schedule an appointment, you are given/emailed a Payment Options Handout which clearly explains rates and potential discounts for you. You will be able to select which option you want during your initial appointment.

Insurance

I participate with insurance companies in the following way. I am an out-of-network provider. If you pay full fee, you can request a “Superbill” (invoice) that you can send to your insurance company to receive reimbursement if you have out-of-network benefits and you have paid your deductible.

Cancellation Policy

If you do not attend your scheduled appointment, and you have not notified me at least 12 hours in advance, you will be required to pay the full cost of the session.

Payment

Payment is due at the time services are rendered. American Express, Visa, MasterCard, Discover/Novus, cash, and checks are accepted. Crime Victim’s Compensation.

If you desire, you can print and complete the first three forms (not HIPAA) and bring them to to your initial appointment. Otherwise, arrive 15 minutes early for that appointment and there will be paper work for you to complete.
